We are looking for a Service Manager - Dual Diagnosis Partnerships to join our team.

The Service Manager - Dual Diagnosis Partnerships will be responsible for managing dual diagnosis partnerships, which includes the Bounce Back project and the Complex Needs Housing Team. The aim of both services is to support people with multiple needs such as mental health, drug and alcohol issues, housing and homelessness, physical health and domestic abuse issues.

This role forms part of the Complex Needs and Community Team at Hertfordshire Mind Network. This is a new role and will be responsible for managing dual diagnosis partnerships, which includes the Bounce Back project and the Complex Needs Housing Team. The aim of both services is to support people with multiple needs such as mental health, drug and alcohol issues, housing and homelessness, physical health and domestic abuse issues.

The Bounce Back project works in inpatient wards as well as A&E departments across Hertfordshire. This project aims to facilitate hospital discharge and prevent further readmission by providing a community support offer, following people in their journey from hospital back home.

The Complex Needs Housing Multi-Disciplinary Teams project works with district and borough councils and is for people presenting as homeless with a mental health need. Both projects work in partnership other agencies. Complex needs' are defined by the National Complex Needs Alliance as a combination of health needs and social needs.

The purpose of the Hertfordshire Mind Network Dual Diagnosis Services Manager role is to:

  • To support the Head of Complex Needs and Community with effective running of the services.
  • Attending contractual and strategic meetings.
  • Line manage staff and where necessary volunteers.
  • To maintain and enhance service delivery standards and effectiveness.
  • Monitor outcomes and evaluate the service.
  • Hold responsibility for all quarterly reporting alongside the Team Leaders.
  • Have a strong knowledge in Safeguarding, including ensuring that the safety and wellbeing of clients using the service is monitored and reviewed regularly.
  • Embed a person centred and solution focused approach in all aspects of the role.
  • To work in a trauma informed way including awareness of the six principles of trauma informed practice, putting the clients emotional and practical needs at the forefront of this work.
  • Ensure all KPIs are met.
  • Building strong relationships and liaising with commissioners.
  • To remain a source of independent support for all clients.
